The U. S. Food and Drug Administration (FDA) last week approved the first treatment to treat high cholesterol.
The drug, called Viagra, was originally developed by Pfizer in the early 1990s as an alternative to diet pills for the heart, but it quickly became a go-to treatment for high cholesterol.
The FDA approved the drug in 2003, but only under the supervision of a lab test, said Dr. Mark P. Dolan, the FDA's director of cardiovascular, cardiovascular and oncology research.
“We thought it was important to have this information on the label, so we did have some discussion around it,” said Dr. P. M. Dolan, who oversees the agency’s laboratory testing and production.
“But it didn’t seem to be that straightforward. It was pretty straightforward. We started with the very first drug and the first drug has been approved.”
The drug works by dilating blood vessels, which dilates the blood supply to the heart and lungs. A drug like Viagra can cause a heart attack or stroke if left untreated, too.
In order to reduce the risk of a heart attack or stroke, doctors usually perform a heart MRI or blood tests before the drug is started.
A small number of patients will have the same results at the same time, said Dr.
The FDA typically takes blood tests after the drug is approved and uses a blood sample, as well as a chest x-ray or ultrasound.
“That’s one of the things we want to focus on,” he said. “So, if you have a blood test, we want to do that.”
While the drug is not a heart attack or stroke medication, it does reduce the risk of heart attacks and strokes, particularly when used long-term.
“We’re trying to have people on these medications and they’re going to be very vulnerable,” said Dr. Dolan, director of the Division of Cardiovascular Prevention and Treatment at Stanford University.
He said some people are more susceptible to a heart attack or stroke than others, and they’re often more vulnerable to heart attacks and strokes, especially if they take a statin like atorvastatin, which is used to treat high cholesterol.
“And this is one of the main reasons it’s a big issue,” he said.
Viagra, known generically as sildenafil, was originally developed to treat angina, but doctors found it wasn't an effective treatment for angina.
The FDA approved the drug in the early 1990s, and doctors then turned to the off-label use of sildenafil. In 2003, the drug was approved in Europe, and then in 2006, as an alternative to diet pills.
In Europe, sildenafil is used to treat erectile dysfunction (impotence) or low blood pressure, and patients are then offered a placebo for a few weeks. The placebo can be taken as a pill or a tablet, but patients should be assessed before taking a pill, said Dr. Dolan.
In the United States, sildenafil is used to treat erectile dysfunction, which is a condition in which the penis is unable to make enough blood to pump blood into the penis, and can cause an erection. The drug is approved to treat high blood pressure, but doctors typically must check that blood pressure is controlled before taking the drug.
“It’s pretty straightforward.”
Viagra is available as a pill or as a liquid, and it works by relaxing the muscles in the blood vessels of the penis.
The FDA does not recommend long-term treatment for erectile dysfunction, and it is not approved to treat high blood pressure or chest pain.
“We don’t want to give people a Viagra that they can’t use in the first place,” said Dr.
“I just don’t want to do that because I know the risk is going to be higher,” he said.
